In a large bowl, combine brown sugar, Dijon mustard, maple syrup, butter, and Worcestershire sauce until well mixed and smooth.
Place your ham in the center of the crockpot and pour the apple juice around the ham.
Pour your brown sugar mixture over your ham making sure to coat the entire ham. You may need to spoon or brush the mixture over the ham to make sure it is covered.
Cover your slow cooker and cook on low for 3 hours.
Once cooked, remove the ham from the crockpot but reserve the sauce to brush over the ham as it is served, for added flavor.
Slice and serve warm.
Notes
There are many things you can do with leftover ham. Turn it into a ham and cheese omelet for breakfast, use it in a linguini pasta meal, or use the leftovers to make ham croquettes.
Store any remaining ham in an airtight container in the fridge for up to 4 days. You can heat the ham at a low temperature or serve it cold. Save the glaze in a separate container so you can apply the glaze over the ham or use it in other ways. Reheat the glaze in a saucepan.
